Diabetes Management Program

MRPT Diabetes Program provides a multidisciplinary approach in diabetes care through medically supervised exercise, nutritional counseling and education. Our goal is to assist those with diabetes, make a life long commitment to sensible exercise, good nutrition, and informed medication management so they may become more active partners with their physicians in turn improving their quality of life. 

Who can participate?

Individuals with any of the following:

  • Pre Diabetes

  • Type I Diabetes Mellitus

  • Type II Diabetes Mellitus

What are the requirements to participate in the program?

You must be referred by your physician and have had an exercise stress test completed within the past year. The stress test will identify your current health status and aid in the development of your individualized exercise prescription.  Mountain River Physical Therapy requirements are based on the American College of Sports Medicine (ACSM) guidelines.

What are the components of the program? 

  • Individualized Training Sessions

  • Body Composition Analysis

  • Blood Glucose Monitoring Daily

  • Blood Pressure Monitoring Daily

  • Nutritional Counseling

  • Progress Updates to Referring Physician
